P22 Implementing GREATix: learning from excellence in paediatric services

Aim The paediatric service is extremely good at reporting incidents through Datix system. These are reviewed, learning identified and then information disseminated, or even a change in practice takes place, all with the aim to improve things prevent errors happening. A regular bulletin prepared for multidisciplinary team pharmacist involved medication section of this bulletin. There no system within Trust that happening it was felt department were missing out on from these. Learning Excellence 1 not new numerous areas, both locally nationally have introduced some form. project identify learn everyday excellence as we currently error via Datix, also staff morale. We wanted an anonymised way let colleague know they done great job. ultimate goal would be interrogate report findings. Method oversight group set up by educator. questionnaire issued staff, examining Resources developed support reporting, data collection nominee feedback. Following meetings discussions IQI team, GREATix into paediatrics. pilot initially carried one small area paediatrics, few changes made throughout trust Nominees certificate letter head clinical director. Results Over past 20 months there been total 130 nominations date. themes coded including evidence teamwork communication, putting child family centre care acting positively affect patient safety preserve further harm when incident had occurred. has feedback nominees saying how delighted receiving award, their day, grateful are. Conclusion For those who received nomination increase areas paediatrics embraced more than other areas.
